Requirements

  • RN licensure in the state where the hospital resides required.
  • Social workers may also be accepted in lieu of RN licensure, if they are licensed or hold a bachelor's degree in social work, as required and in accordance with state regulations.
  • MSW, LSW, PT, OT, SLP and RT licensure may be accepted in accordance with state regulations.
  • Current BLS certification required.
  • Three to five years of case management experience required.
  • Prior management experience preferred.

Nice To Haves

  • BSN/MSN preferred.
  • Certification in an approved Case Management Program preferred.
  • Prior experience in IRF or LTACH preferred.

Responsibilities

  • Coordination of the hospital-wide case management program.
  • Overseeing review of patient’s medical record to ensure proper utilization of hospital services.
  • Assisting case managers in providing timely coordination of care and discharge planning.
  • Assuring screening of all admissions to determine the appropriate level of care.
  • Supervising case managers, overseeing the daily operations of the case management department to facilitate optimal financial and clinical outcomes.
  • Fiscal planning and regulatory compliance.
  • Analyzing current systems and variance to identify opportunities for improvement.
  • Promoting quality of care through collaboration with members of the interdisciplinary team.
