About The Position

AccessCNY is seeking a proactive employee to lead the Critical Time Intervention (CTI) model and Transitional Residential Services (TRS) for youth experiencing homelessness. This role involves overseeing operations, ensuring staff meet client needs, managing documentation, and collaborating with external agencies. The ideal candidate will use their experience and judgment to uphold the mission of the CTI model and TRS, supporting youth and families in achieving stability and reaching their full potential within the community.

Requirements

Master degree in Mental Health Counseling, Counseling Psychology or related field, required.
NYS Licensed Master Social Worker or Licensed Mental Health Counselor certification, required.
Must have a valid NYS Driver License in accordance with the Agency Vehicle and Driving policy and transportation necessary to meet program needs.

Nice To Haves

Licensed Clinical Social Worker certification, preferred.
One year of experience working with youth with a mental health diagnosis, and/or intellectual/developmental disability (I/DD) dual diagnosis, preferred.
Case management experience working with youth and families, preferred.

Responsibilities

Oversee referral and workflow for the CTI program.
Ensure CTI staff meet with referred youth and families from hospitals.
Review intake assessments to determine needs and goals, and assist in identifying additional services for stability.
Review visit schedules with youth/families to ensure they meet CTI requirements and client needs.
Monitor the provision of services at the TRS site.
Review progress with supports/referrals and make recommendations for changes.
Supervise the CTI team, TRS Manager, and TRS clinical staff.
Complete and ensure the completion of staff performance evaluations (initial, 6-month, annual), and assist with performance concerns and disciplinary actions.
Ensure all required documentation (progress notes, safety plans, intake/discharge plans, assessments) is completed accurately and maintained per Agency procedures/NYS regulations.
Ensure timely completion of incident reports and implementation of safeguards and recommendations.
Complete and analyze youth/family satisfaction surveys.
Implement systems to measure quality initiatives and program outcomes, monitoring data submission for the Agency dashboard.
Review monthly financial reports and collaborate with supervisor and Finance to adjust the budget/expenses.
Ensure youth rights are protected regarding their money and personal possessions.
Meet with local hospitals to educate them on the CTI process.
Participate in meetings with OMH and follow up on recommendations.
Conduct self in a professional manner.
Provide person-centered, quality services.
Observe confidentiality and safeguards regarding all participant-related information.
Ensure compliance with federal, state, and local employment laws and regulations, as well as company policies.
Perform Other Duties as Assigned.
