About The Position

The Transition & Intake Director at Life Unlimited will play a pivotal role in ensuring that adults with developmental disabilities receive comprehensive and personalized support services. The Transition & Intake Director is responsible for supervising Service Navigators (SNs), and ensure the department guides individuals through the agency's array of services, facilitating access to resources, and fostering a supportive environment that empowers individuals to achieve their goals and lead fulfilling lives. Additionally, the role involves establishing and nurturing connections with local community partners to promote the agency's mission and raise community awareness of that mission.

Requirements

Bachelor’s Degree in a Human Service Field preferred
Minimum of 5 year’s management experience with one (1) of those years working with individuals with developmental disabilities strongly preferred.
Must obtain and maintain training and certification in all required areas such as Abuse/Neglect, CPR, First Aid, Level I Medication Administration, Positive Behavior Supports, TOOLS and Crisis Intervention.
Background in the skills of developmental, implementation and the writing of teaching strategies.
Must have reliable personal vehicle for transporting clients.
Must obtain a Class E driver’s license during new hire orientation; must carry minimum coverage applicable by law for auto insurance
Must Receive Seasonal Flu Vaccination
Must possess good communication skills both orally and in written form.
Must have ability to use computer and other electronic equipment.
Must have a desire to manage a team of staff supporting individuals with developmental disabilities and advocate for them in any possible way.
Must possess self-motivation, initiative and independence to work without direct supervision.
Must be flexible, reliable and dependable.
Must be able to manage and embrace change, look for ways to improve own performance and accept feedback.
Must be willing to accept challenges of a 24-hour support system and provide aid to team(s) in crisis’s.
Ability to plan and prioritize in order to meet deadlines.
Must be willing to perform duties that are assigned outside of this job description.
The ability to effectively and efficiently use computers or mobile devices to input, access, modify, or output information. This includes the ability to quickly access, store and retrieve data.

Responsibilities

Establish and maintain Intake process and corresponding documentation within agency. Ensure process consists of checklists and QA points to ensure all tasks are completed for any situation or program intake.
Establish centralized electronic location for intake process and documentation (i.e Therap, Sharepoint, Teams) ensure all necessary parties can access and view information as needed.
Monitor utilization of intake process documentation by SNs.
Monitor current Intake Waitlist to ensure data is current. Analyze data in Intake Waitlist monthly for service inquiry and trends and present data to Program VP’s and COO.
Act as main liaison to navigate individuals through the various services offered by the agency, ensuring seamless access to programs, therapies, and community resources.
Develop and maintain procedures within the department to ensure comprehensive assessment of individuals' needs, strengths, and preferences is completed during intake process. Ensure data compiled during intake assessments are visible to necessary team members and documented for all future team members.
Ensures individuals have established individualized service plans and with achievable goals for any Life Unlimited services within 90 days of intake.
Coordinate and advocate for necessary services, including medical care, educational support, vocational training, and social activities. Ensure connection to these services occurs within a timely manner.
Serve as an advocate for individuals, ensuring their voices are heard in decision-making processes.
Empower individuals to make informed choices about their lives, fostering independence and self-determination.
Demonstrate person-centered philosophy and language when discussing service and placement options.
Identify potential partnerships with local schools, medical facilities, nursing homes, and other community resources.
Educate community partners on the agency mission and scope of services; integrate agency staff into transition processes with community partner agencies for individuals (i.e. IEP and transition meetings, discharge meetings, etc).
Lead the weekly agency intake meeting with all agency departments. Establish intake timeline expectations; Ensure placements progress within expected timelines across all departments.
Develop and maintain procedures to ensure daily monitoring of the Consumer Referral Database and quick correspondence with individuals and team members which promote placement activity.
Provide supervision and support to Service Navigators between weekly intake meetings explore potential placements, anticipate program questions/concerns, and gather all necessary information prior the weekly intake meetings with all agency departments.
Track and trend all referrals and intake work within the agency. Report out data and analysis to Program VP’s and COO monthly.
Provide supervision and guidance to team members within the Service Navigation Department.
Establish processes within the agency to facilitate smooth transitions into Life Unlimited services. Identify transition points within the intake process and establish roles and responsibilities.
Oversee the intake process, ensuring efficiency and accuracy in assessing individuals needs and directing them to appropriate services.
Maintain and implement all necessary forms for the department, in conjunction with QA and IT ensure necessary data is published and viewable to enable assessment of QA and growth.
Work closely with support departments (Housing, Finance, Training) to ensure all team members and informed of referrals or upcoming placements. Incorporate support department needs into intake checklists and procedures.
Foster strong communication within and between all programs and the overall organization.
Conduct training sessions for individuals, families, and caregivers on available services, self-advocacy, and community resources.
Establish and maintain strong, frequent communication with all individuals or team members inquiring about services at Life Unlimited. Follow contact until resolution of need.
Along with the Service Navigators, connect individual and team members to appropriate agency personnel. Follow up on connections until services begin and the individual is fully integrated into programs.
Establish and maintain strong relationships with families, caregivers, and external service providers to create a comprehensive support network.
Collaborate with interdisciplinary teams to ensure a holistic approach to individualized care.
