Direct Support Worker - Caregiver

GT Independence Careers•Canyonville, OR

About The Position

GT Independence is seeking compassionate and reliable Direct Support Workers/Caregivers to support individuals who self-direct their services. The worker provides one-on-one assistance in the individual’s home and community, helping them live as independently as possible while respecting their choices, preferences, and goals. The role follows a person-centered, self-directed model: the individual receiving services directs their own care, and schedules and tasks are often tailored to that individual rather than to a facility or agency. Support includes personal care, mobility, meals, household routines, community participation, and health-related needs as directed and authorized. Workers follow the individual’s Service Plan and approved tasks, accurately document services, complete required training, and comply with program guidelines.
