About The Position

Mosaic is seeking a Direct Support Specialist to support people with intellectual and developmental disabilities in living full lives. The position serves four people in independent living, all of whom live in their own apartments, through scheduled visits. The role includes helping with daily activities, community access, health-related needs, and progress toward goals in each person’s Individualized Service Plan.

Requirements

• High school diploma or equivalent.
• At least one year of experience in a related field.
• Must be at least 18 years of age.

Responsibilities

• Attend scheduled visits with four individuals in independent living.
• Assist people served with handling money and finances.
• Provide transportation and help people served access services in the community.
• Assist with general housekeeping, grocery shopping, meal preparation, and laundry.
• Escort individuals to medical, dental, or therapy appointments and assist them in communicating with healthcare providers.
• Monitor medications and/or administer prescribed medications and treatments.
• Maintain accurate daily shift logs and track progress toward goals outlined in each individual’s Individualized Service Plan.
