About The Position

Under general supervision of the Site Leader, performs duties related to the overall care of individuals in assigned residential facilities. Must be able to successfully complete all of the functions and competencies associated with this position as well as other duties as they may be assigned. Open shifts are as follows: #2 - Part-time / Nights: 10:30p to 8:30a / 60 hours bi-weekly Week 1: Thursday, Friday & Saturday Week 2: Thursday, Friday & Saturday #4 - Part-time / Variable Shift: Days & Evenings / 42 hours bi-weekly (willing to split) Week 1: Sunday (8:30a - 2:30p), Monday (1:30p - 10:30p) & Saturday (8:30a - 2:30p) Week 2: Sunday (8:30a - 2:30p), Monday (1:30p - 10:30p) & Saturday (8:30a - 2:30p) #5 - Part-time / Every Weekend: 2:30p - 10:30p / 32 hours bi-weekly Week 1: Sunday & Saturday Week 2: Sunday & Saturday

Requirements

  • Minimum of a High School Diploma or GED and 1 year of work experience preferably in a care providing role, or exposure to individuals with developmental or intellectual disabilities or a combination of experience and education
  • Basic level of computer proficiency as well as good basic reading and writing skills
  • Excellent communication, interpersonal charting and record keeping skills
  • Ability to accept and provide direction and instruction in a professional manner
  • Successfully pass a background check that includes a BCI and FBI criminal history check
  • Must maintain current Med Pass and CPR/First Aid Certification; training available for CPR and Med Pass onsite
  • Must be a valid driver with a current Ohio Driver’s License reflecting less than 6 points and proof of state minimum vehicle/general liability insurance coverage.

Responsibilities

  • Come to work on time, in the proper attire prepared to work
  • Make sure all required training and certifications, licenses and insurance requirements are up to date
  • Adjust work hours as required for effective operation of the program
  • Providing individuals with assistance in everyday life skills
  • Provide active treatment, emotional and health-related services including administering medications, assisting in breathing treatments, checking vitals to monitor overall health and assisting with medical equipment.
  • Assist with providing basic personal care and grooming, including bathing, basic hair care and getting dressed
  • Responsible for administrative task associated with providing support.
  • Accurately administering medication and following and maintaining MARS
  • Immediately report any incidents or accidents involving individuals to the Site Leader
  • Accurately, legibly and cohesively document all services provided to individuals
  • Maintain a clean and safe facility and always ensure the safety and wellbeing of the individuals
  • Involve individuals in community and recreation-based activities
  • Assist with and or complete household chores such as cleaning, laundry, cooking, vacuuming, and others
  • Make and accompany individuals to appointments and community events
  • Perform and record physical assessments including taking and recording vital signs and medical histories
  • Exhibit acceptable work standards and immediately correct actions that do not meet Agency standards.
  • Ensure completion of required annual training by established deadlines.
  • Follow Homemaker and Personal Care (HPC) electronic and paper documentation.
  • Make sure that ISP’s and Behavior Support Plans are followed
  • Report UIR and MUI situations to Site Leader according to guidelines provided
  • Keeping and documenting records of body and general living conditions
  • Transport individuals in Agency vehicles to various locations in the community. This may also include transporting individuals who do not reside at residential site.
  • All other related tasks as assigned by management.
