Requirements

  • High School Diploma or GED
  • Valid Driver's License
  • Basic computer and internet proficiency
  • Ability to read, write, and document effectively in English
  • Ability to complete and maintain required certifications and trainings

  • Prior experience in human services, healthcare, behavioral health, or developmental disabilities services

Responsibilities

  • Assist individuals with activities of daily living, including personal hygiene, grooming, dressing, meal preparation, and household tasks.
  • Promote independence and skill development while providing support as needed.
  • Encourage participation in home, recreational, vocational, and community activities.
  • Foster a safe, supportive, and respectful environment.
  • Support individuals in participating fully in their communities.
  • Transport individuals to appointments, day programs, recreational activities, family visits, and community events.
  • Assist individuals in developing social relationships and community connections.
  • Support individuals in achieving goals outlined in their Individual Support Plans (ISPs).
  • Participate in ISP development and implementation.
  • Follow behavior support plans and supervision requirements.
  • Document progress toward goals and outcomes.
  • Administer medications according to agency policies and state regulations.
  • Monitor individual health and wellness.
  • Report concerns, incidents, and changes in condition promptly.
  • Complete required documentation accurately and timely, including daily logs, Medication Administration Records (MARs), goal tracking documentation, incident reports, behavioral documentation, vehicle logs, consumer fund documentation, and other required records.
  • Maintain confidentiality and comply with HIPAA requirements.
  • Respond appropriately to crisis situations.
  • Follow incident management and reporting procedures.
  • Report suspected abuse, neglect, exploitation, or mistreatment immediately.
  • Assist in maintaining the health, safety, and well-being of all individuals served.
  • Attend staff meetings, trainings, and required agency events.
  • Participate in ongoing professional development.
  • Support on-call and emergency coverage as needed.
  • Work collaboratively with supervisors, co-workers, families, and service providers.
