Requirements

High school diploma or equivalent required.
Experience in direct support, disability services, or related areas.
Driver's License (Required)

Responsibilities

Provide person-centered support in a private home and the community
Implement Individual Support Plans, behavior supports, and therapeutic routines
Assist with life skills, household responsibilities, and community activities
Coordinate medical appointments and support overall health and wellness
Collaborate with families, caregivers, clinicians, and team members
Document services and maintain confidential records in accordance with agency standards
