About The Position

This position involves implementing interventions for individuals in crisis, under the supervision of a behavior specialist and lead LCSW/LPC, for In-home and Out-home Crisis Support Services. The role requires maintaining hyper-vigilance during shifts to provide redirection and behavioral interventions, acting as an effective adult model, and implementing Individual Support Plan (ISP) goals. Responsibilities include accurate daily documentation of activities, drills, behavior support plans, medication monitoring, and ISP tracking. The role also involves reviewing medication administration reports, assisting with creating a conducive physical environment through cleaning and laundry, executing Activities of Daily Living (ADLs), communicating professionally, and participating in annual trainings. Immediate notification of elopements or aggressive behaviors to the supervisor is critical. The position also includes developing weekly/monthly menus with individual assistance, monitoring vehicle condition, managing aggressive behaviors, and other assigned duties. Travel may be required.

Requirements

  • Must be 18 years of age or older.
  • Possess a valid driver’s license.
  • Possess valid insurance with liability or specific plan for transportation, if required.
  • Minimum of three (3) years’ experience working with Individuals with Mental Health diagnoses.
  • Minimum of three (3) years’ experience working with Individuals exhibiting challenging behaviors.
  • Minimum of three (3) years’ experience working with Individuals with Individual and Developmental Disabilities (IDD).
  • Demonstrate strong interpersonal skills.
  • Demonstrate the ability to implement supportive strategies.
  • Demonstrate a commitment to promoting independence and quality of life for the individuals served.
  • Successfully pass an E-Verify check.

  • Participate in annual trainings to remain updated on skills and as needed based upon the behavior support plan.

Responsibilities

  • Implements interventions to persons in crisis under the supervision of the behavior specialist and lead LCSW/LPC for In-home and Out-home Crisis Support Services.
  • Maintains hyper-vigilance during shifts to provide redirection and other behavioral interventions to individuals in crisis.
  • Demonstrates and performs as an effective adult model for people supported.
  • Implements ISP goals.
  • Keeps accurate daily documentation of activities (progress notes), fire/disaster drills, behavior support plans, monitors and tracks targeted behaviors, medications, ISP tracking, and other documentation, entering it into the documentation system per protocols.
  • Reviews medication administration reports for assigned homes and supervises individuals with self-administered medications.
  • Assists in creating a physical environment that lends to progress, including house cleaning and laundry.
  • Assists with the execution of ADLs.
  • Communicates professionally and acts respectfully with people served and supported.
  • Participates in annual trainings to remain updated on skills and as needed based upon the behavior support plan.
  • Notifies supervisor of individuals' elopements or physically aggressive behaviors immediately.
  • Develops weekly/monthly menu with individual assistance.
  • Monitors the vehicles assigned to the homes for cleanliness and good working order.
  • Manages aggressive behaviors.
