Requirements

High school diploma or GED.
Basic computer and internet proficiency, including the ability to use email, electronic documentation systems, and timekeeping platforms.
Valid driver's license and acceptable driving record.
Ability to successfully complete all required background checks and employment screenings, including FBI clearance where applicable.

Nice To Haves

Associate's or Bachelor's degree in Human Services, Psychology, Social Work, or a related field.
Previous experience in mental health, behavioral health, residential services, or other human services settings.

Responsibilities

Provide supervision and support in accordance with each individual's ISP and person-centered support plan.
Assist individuals in achieving personal goals and desired outcomes.
Complete required documentation accurately and within established timeframes.
Administer medications in accordance with agency policies and procedures.
Provide transportation to appointments, community activities, and other approved destinations.
Support individuals with activities of daily living, including: Meal preparation, Personal hygiene, Toileting and personal care, Household tasks, Other individualized support needs
Assist individuals with community integration and access to resources, including: Medical appointments, Social services, Recreational and leisure activities, Community-based supports and services
Promote independence, dignity, self-advocacy, and positive social interactions.
Maintain a safe and supportive living environment.
Perform other duties as assigned by supervisory staff.
