About The Position

The Direct Home Care Worker provides supportive services to participants in various settings, including their homes, the community, during transport, and at appointments or the day health center. Their responsibilities are detailed in the participant's Direct Home Care Worker Care Plan, which is based on assessments by a home care RNCM. The role involves assisting frail elders with personal care, daily activities, meal preparation, toileting, simple treatments, shopping, medication reminders, laundry, homemaking, and transportation. Accurate and timely documentation of services provided is crucial, as is keeping the RNCM and clinic nurse informed of participant progress. The worker must also report deviations from schedules and appointments to their supervisor and respond to emergencies according to policy, while operating within their scope of practice.

Requirements

  • Minimum High School Diploma or G.E.D. required.
  • Minimum of (2) years of relevant work experience.
  • Completed formal training program (Direct Care, Nurse Assistant, Medical Assistant, etc.) outside of PACE SEMI in lieu of minimum years of experience.
  • Participate in annual, required in-service training and screening, including infection control, TB testing, safety training, and BLS training.
  • Ability to establish and maintain interpersonal and interdepartmental relationships.
  • Respect the physical, spiritual, and well being of participants and co-workers.
  • Ability to organize and maintain flexible scheduling to coordinate job responsibilities.
  • Ability to work sensitively with individuals of diverse ethnic and cultural backgrounds.
  • Ability to relate to those participants with dementia and/or developmental disabilities.
  • Ability to perform duties in an atmosphere of frequent interruption.
  • Meet a standardized set of competencies (approval by CMS) before working independently.
  • Medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
  • Driving is required within PACE SEMI catchment area.
  • Must have reliable transportation available on a daily basis.
  • Must have current valid driver’s license.

Responsibilities

  • Provide supportive services to participants in home or community settings, during transport, and on appointments.
  • Assist in providing and/or maintaining optimal physical and emotional comforts to frail elders.
  • Assist participants and their families toward achieving maximum self-reliance and independence.
  • Perform personal care, assistance with activities, meal preparation/feeding, toileting/managing incontinence, simple treatments, shopping, medication reminders, laundry services, extended hours care, homemaking services/chores, and transportation/off-site appointment support.
  • Perform accurate and timely documentation of care and/or services provided on every visit and/or encounter.
  • Keep the RNCM and/or clinic nurse informed of the participant’s progress.
  • Immediately report all customer service complaints and all deviations from set schedule and appointments to supervisor/manager.
  • Respond to emergency situations in accordance with established policies and procedures.
  • Work only within established scope of practice.
