About The Position

The Direct Care Worker provides direct assistance and support to individuals with developmental disabilities. The role includes helping with personal care and daily living activities, implementing skill-building and Individualized Support Plan (ISP) goals, supporting community participation, and offering companionship and supervision. The worker also completes service documentation and reports changes in condition or incidents to the LifeCare Supervisor or after-hours coordinator.

Requirements

• High school diploma or GED.
• At least 3 months of caregiving or related experience.
• Ability to read, write, and communicate effectively in English; an interpreter is required if the worker cannot do so.
• Skills supporting individuals with developmental disabilities.
• Familiarity with ISP documentation and goal-setting.
• Must be at least 18 years of age.
• AHCCCS DCW Training or current CNA/LNA (AZBN).
• DCW Developmental Disability Training Module.
• Skill Building or Habilitation/ISP Training.
• Article 9 Certification.
• Prevention & Support Training, if applicable.
• Current CPR and First Aid Certification.
• DPS Level 1 Fingerprint Clearance, or ability to obtain it within 90 days of hire.
• APS and Central Registry Check.
• Notarized Criminal History Affidavit, initially and annually.

Nice To Haves

• At least 1 year of caregiving or related experience.
• Bilingual skills.

Responsibilities

• Provide support with personal hygiene, meal preparation, housekeeping, mobility, and feeding.
• Implement skill-building activities and ISP goals.
• Support participation in errands, recreation, and community-based learning.
• Promote social connection and provide supervision and emotional support.
• Complete daily records and EVV entries accurately, reflecting services provided and progress toward goals.
• Report changes in condition or incidents to the LifeCare Supervisor or after-hours coordinator.
