About The Position

The Provider, Direct Care, will implement and maintain a Community Integration program for all assigned consumers to achieve their individual goals to provide intensive support services to develop the abilities necessary to maximize their independent functioning in a home and community environment.

Requirements

  • Must be able to write clear, concise, and accurate reports and plans, submit them in a timely fashion.
  • Possess excellent time management skills.
  • Must have a working knowledge of community resources.
  • Must conduct self in a professional manner and maintain a professional appearance.
  • Must have the ability to work independently, cooperatively and to exercise effective planning.
  • Must demonstrate flexibility within the framework and pressure of established deadlines.
  • Must have the ability to drive for company business.
  • Must have valid Colorado State driver license.
  • Must also be able to operate company vehicle, and adhere to all Federal, State, and local laws governing vehicle operation.
  • Applicants/employees using their personal vehicle for company business must have valid vehicle insurance, valid driver license, and vehicle must be in roadworthy condition.

Nice To Haves

  • A High School Diploma or equivalent is preferred.
  • Experience in related field or equivalent combination of training and experience in a social service, counseling, or rehabilitation setting, providing direct services to persons with physical, emotional and/or developmental disabilities.
  • Excellent written and verbal communication skills.
  • Demonstrate the ability to interact with various types of internal and external customers.
  • Train and be creative in motivating consumers toward individual success.
  • Ability to relate effectively to disabled person, their parents and guardians and be empathetic toward their needs.

Responsibilities

  • Meet with potential new clients to interview as a potential provider.
  • Complete all client program paperwork to include Individual Support Program (ISP) tracking, monthly timesheets, incident reports.
  • Provide services specified in the Individual’s Plan and assisting them with working towards their goals and documenting the progress.
  • Complete all necessary client specific documentation, e.g., timesheets, log notes, ISP tracking, and incident reports on a monthly basis.
  • Maintain complete and accurate records and statistical data as required for all clients and programs.
  • Participate in required training sessions.
  • Deal effectively and calmly with non-routine situations following accepted guidelines and perform other duties as required by the Program Coordinator.
  • Follow all guidelines set forth by state regulations and will ensure that quality communication occurs between them, the client, and Goodwill of Colorado Staff.
  • Perform direct service tasks outlined in the plan of care, such as; personal care, emotional support, housekeeping, and home management.
