About The Position

Harbor Health's Elder Service Plan/PACE Program is seeking a full-time Dietitian to join their dedicated Nutrition Team. This role focuses on helping older adults live healthier, more independent lives by managing the nutrition program for the Elder Service Plan. Harbor Health operates two Programs for All-Inclusive Care for the Elderly (PACE) in Mattapan and Brockton, providing comprehensive healthcare, transportation, and social services to over 580 frail elders, enabling them to live with dignity and independence in the community. The position offers a hybrid work model, including on-site work, home visits within the community with mileage reimbursement, and remote work.

Requirements

Bachelor’s degree from an accredited college with major studies in Food and Nutrition or Dietetics.
Current License in the state of Massachusetts as a Registered Dietitian.
Current BLS (Basic Life Support for Healthcare Providers) Certification.
Intermediate skills with Microsoft Office 365 including Word, Excel, Outlook and Teams.
One year of professional experience providing nutrition counseling and assessments.
One year of experience working in geriatrics required.
Excellent written and oral communication skills.
Current ServSafe – Allergen OR MA-Allergen Certification or obtained within 3 months of employment.
Current ServSafe Food Handler Certification (Food Handlers or Serve Safe Manager) or obtained within 3 months of employment.

Master’s degree preferred.

Responsibilities

Implements the overall nutrition program for the Elder Service Plan.
Ensures nutrition programs comply with regulatory and compliance agencies and meals meet PACE and DPH requirements.
Provides guidance and advice to participants and their families regarding nutrition.
Assists in overseeing food services programs provided by food vendors.
Actively participates in interdisciplinary teams (IDT).
Performs and documents timely initial and annual nutrition assessments, including diet order, height and weight, lab results, dietary history, preferences, medical conditions, and medications affecting nutrition.
Develops individual nutrition plans for participants and communicates them to the participant, IDT, Dietary Aides, family members, and other caregivers.
Communicates concerns or issues arising from Nutrition Assessment to the IDT.
Ensures the Nutrition Plan is implemented by providing education materials and one-on-one guidance, and by guiding Dietary Aides, Center Aides, and Home Health Aides.
Recommends and ensures the provision of nutrition supplements when appropriate, communicates needs to the IDT, and monitors participant use.
Recommends and arranges for home-delivered meals for participants as needed.
Arranges for and supervises the provision of specialized meals or supplements when necessary.
Assists the Nutrition Manager in ensuring Dietary Aides perform necessary functions, including food temperature control, equipment review, log maintenance, and compliance with regulations (OSHA, DPH, PACE, CMS).
Utilizes EMR and subjective information for comprehensive and accurate nutrition assessments.
Assists in presenting Annual Safety training for ESP staff on food handling, temperatures, feeding, and kitchen sanitation.
Provides functional supervision to Dietary Aides during Adult Day Health Center kitchen operations.
