Our multi-specialty group practice offers a broad range of medical sub-specialties. We are seeking a full-time 40-hour Diabetes Program Coordinator to support our Internal Medicine suite Monday through Friday from 8:00-5:00pm. The schedule consists of 4 days in Haverhill and 1 day at our Andover location.

Requirements

Bachelor's Degree Related Field of Study required
Health Education &/or Literacy Experience 3-5 years required

Nice To Haves

Registered Nurse [RN - State License] - Generic - HR Only preferred
Basic Life Support [BLS Certification] - Various Issuers preferred
Certified Diabetes Care and Education Specialist [CDE/CDCES] - Certification Board for Diabetes Care and Education (CBDCE) preferred

Responsibilities

Maintain accreditation for the program.
Conduct individual and group patient education sessions; maintain the schedule of patient appointments.
Coordinate reporting for audits and implement recommendations as a result of the audits.
Maintain records pertaining to program activities.
Purchase or approve the purchase of equipment and supplies needed by the staff and program.
Schedule program work, oversee daily operations, coordinate the activities of the program, and set priorities for managing the program.
Manage marketing and outreach to the community.
Prepare or assist in the preparation of grant requests and funding continuation from outside sponsors.
