The Diabetes Program Coordinator is responsible for the administration of diabetes case management and program development to reduce the instances of diabetes and health complications. This includes all objectives of applicable diabetes grant deliverables, public health policy, and priorities of the Nisqually Tribal Health Center. The role encompasses areas of diabetes prevention, nutrition, patient and staff education, patient and program assessment, patient self-management skills, and connecting patients to supplemental resources.
