About The Position

Performs in-depth assessments and the formulation of individual plans of care for people with diabetes. Acts as a resource to professional staff and the community, providing outpatient diabetes education. Participates in hospital and community in-services and support groups. Investigates grant funding for projects associated with diabetes and chronic conditions associated with diabetes.

Requirements

• Graduate from an accredited school of nursing with current license to practice in the State of Wisconsin required.
• Registered Dietician also acceptable.
• BLS certification required.
• Demonstrates understanding of teaching/learning process, nursing theory and practice.
• Excellent communication and demonstrated understanding of teaching/learning process, nursing theory and practice.
• Must be well organized, flexible, and able to work independently as well as within a team of professionals.

Nice To Haves

• Certified Diabetic Educator (CDE) certification preferred.
• Minimum of three years of experience in field of expertise preferred.
