About The Position

Saint Francis is seeking a full-time Diabetes Nurse Clinician for their Yale Campus in Tulsa, Oklahoma. This role focuses on promoting quality patient and family care through the nursing process, with a specialty in diabetes management. The clinician will provide health care consultation, collaborate with healthcare providers on care plans including discharge planning, and serve as a resource for staff and the community. Responsibilities include facilitating educational activities for patients, families, staff, and the community, developing educational materials, and potentially serving as faculty for nursing schools and hospitals. The role also involves managing resources, accurate reporting and charging, monitoring supplies, and utilizing research to advance nursing practice and promote change.

Requirements

  • Minimum of 2 years of nursing experience with diabetes patient experience strongly preferred.
  • Experience providing diabetes education in an inpatient/outpatient setting and Certified Diabetes Care and Education Specialist (CDCES) certification also preferred.
  • Completed the basic professional curricula of a school of nursing as approved and verified by a state board of nursing, and holds or is entitled to hold a diploma or degree therefrom.
  • Valid multi-state or State of Oklahoma Registered Nurse License.
  • 1 - 2 years related experience.
  • Working knowledge of general disease state management processes, bio-psychosocial model, and clinical pathways.
  • Effective interpersonal, written, and oral communication skills.
  • Ability to organize and prioritize work in an effective and efficient manner.
  • Ability to be detail oriented as required in the examination of clinical and numerical data.

Nice To Haves

  • Diabetes patient experience strongly preferred.
  • Experience providing diabetes education in an inpatient/outpatient setting.
  • Certified Diabetes Care and Education Specialist (CDCES) certification.

Responsibilities

  • Utilizes the nursing process to promote quality patient and family care.
  • Assists in the development, implementation, and evaluation of protocols, policies, and standards of care for the specialty patient.
  • Functions as case manager of special population.
  • Provides health care consultation for area of specialty.
  • Provides patient and family consultation.
  • Consults and collaborates with health care providers to establish and implement plan of care, including discharge planning.
  • Serves as resource for all health care providers.
  • Facilitates educational activities for patients, families, staff and community based on identified needs of specialty population.
  • Identifies, plans, and provides educational services for patient and family.
  • Participates in staff, community outreach, and education.
  • Assists in education literature/tool development.
  • Serves as faculty/consultant to nursing schools and area hospitals.
  • Manages resources in competent manner.
  • Reports and charges encounters and activities accurately and promptly.
  • Monitors supplies and educational inventory.
  • Utilizes research to support and advance nursing practice in area of specialty.
  • Assists in product evaluation and implementation.
  • Communicates research findings and utilizes research to promote change in practices.
