Diabetes Educator (RN), Full Time - Days, Endocrinology, Morristown
About The Position
This full-time position is for a Diabetes Educator (RN) in Endocrinology, working day shifts. The role involves coordinating individualized patient and support system assessment, education, planning, and implementation of care goals. The educator collaborates with referring providers and the interdisciplinary team to achieve optimal patient education outcomes and glycemic control. Maintaining professional education standards for CDE in an ADA-approved Diabetes Self-Management Education program is crucial. The role also includes planning and conducting nutritional programs, assessing nutritional needs considering cultural and educational factors, and providing individualized nutrition education. The Diabetes Educator serves as a resource on nutrition and dietetics, communicates patient information to providers and staff, and documents all educational interactions and evaluations.
Requirements
- Bachelor's Degree in Nursing, Food, Nutrition, Dietetics or related field.
- License to practice as a Registered Professional Nurse.
- Current Diabetes Educator Certification. Or obtain within 1 year of hire.
- 3 years of experience as a Diabetes Educator or relevant.

Responsibilities
- Coordinates individualized patient and support system assessment, education, planning and implementation of care goals.
- Collaborates with the referring providers and interdisciplinary team to work toward optimal patient education outcomes and glycemic control.
- Maintains professional education standards required to be a CDE in a ADA approved Diabetes Self-Management Education program.
- Plans and conducts nutritional programs, under supervision, to assist in the promotion of health and control of disease.
- Assesses nutritional needs and considers patients' cultural, educational, and learning issues.
- Provides individualized nutrition education to patients, their families, and/or significant others based on their needs assessment.
- Serves as a resource on nutrition and dietetics to nurses, health educators or other health care providers in the facility.
- Communicates patient information to referring providers and appropriate staff in a timely manner.
- Documents nutrition information discussed, written materials provided, and evaluation of patient's receptiveness and comprehension of the education provided.
- Performs other related duties as assigned.
