About The Position

This is a full-time, 3.5-year, grant-funded position for a Diabetes Educator. The role involves leading weekly group education classes, delivering diabetes disease state and financial education from an established curriculum. The educator will also provide intensive health, wellness, and financial coaching, as well as disease state management for study participants. Coaching visits are monthly and can be in-person or remote. Comprehensive documentation is required. Additional responsibilities may include administering the food pharmacy benefit, participant recruitment, screening for eligibility, data collection, and data entry. The position is hybrid, allowing up to three days of remote work per week, with one in-person evening session per week for group classes. There are no holidays or weekends required.

Requirements

  • Health Care Professional (PharmD, RN, NP, PA, APRN, RD) licensed in the state of Nebraska.
  • Excellent oral and written communication skills.
  • Ability to speak in public.
  • Ability to counsel patients.
  • Ability to motivate patients toward positive behavior change.
  • Ability to document patient visits.
  • Ability to communicate via email with research team and study participants.
  • Knowledgeable in diabetes pathophysiology and treatment (both lifestyle and pharmacotherapy).

Responsibilities

  • Lead weekly group education classes, delivering diabetes disease state and financial education.

  • Offer disease state management for study participants.
  • Conduct monthly coaching visits, which may occur in-person or remotely.
  • Complete comprehensive documentation for all visits and activities.
  • Administer the food pharmacy benefit.
  • Assist with participant recruitment.
  • Screen participants for eligibility to enroll in the study.
  • Perform data collection and data entry.
