About The Position

The Diabetes Educator Program Coordinator oversees day-to-day operations of the Diabetes Self-Management Education (DSME) program at Froedtert Health outpatient sites, ensuring National Standards for Diabetes Self-Management Education and Support (NSDSME) are met and maintained. The role directs and develops Froedtert Health’s outpatient American Diabetes Association (ADA)-recognized diabetes education program and works as an educator, program coordinator, and evaluator of diabetes-specific outcomes for ADA recognition. The coordinator assists managers in achieving high levels of clinical, operational, and performance excellence, as well as patient satisfaction in the delivery of DSME and training. The role participates in compliance and quality performance improvement efforts, incorporates the nutrition care process, and maintains the professional competency and skills required for professional practice. This is a hybrid position based at the CP-North Hills Health Center in Menomonee Falls, Wisconsin, with a 1.0 FTE and 40 standard hours per week. The stated schedule is 8:00 a.m.–4:30 p.m.; no weekends or holidays are required.

Requirements

At least 3 years of professional clinical experience in diabetes care.
Bachelor’s degree in Food and Nutrition or Dietetics, or a bachelor’s degree in a related field plus certification in Food and Nutrition or Dietetics.
Current Registered Dietitian credential from the Commission on Dietetic Registration.
Current Wisconsin Certified Dietitian credential.
Registration by the American Association of Diabetes Educators as a Certified Diabetes Educator.
American Heart Association Basic Life Support for the Healthcare Provider certification, or an AHA-approved equivalent, within 90 days of hire.
