About The Position

ViaQuest Residential Services provides support and services to individuals with developmental disabilities in their homes. The LPN provides care and day-to-day support to patients, following their plans of care and working with a supervising RN, physicians, and the interdisciplinary team. The position is available part-time or full-time, with a 7 a.m. to 7 p.m. schedule. The listed pay is $28–$29 per hour, plus an additional $4 per hour on weekends.

Requirements

• Minimum of an associate's degree or equivalent in Nursing from an accredited nursing program.
• LPN license.
• Valid driver's license, car insurance, and reliable transportation.

Nice To Haves

• Nursing experience, preferably in Private Duty Nursing, Home Health, or Disability Services; experience is a plus but not required.

Responsibilities

• Perform duties outlined in the plan of care, including medication administration, personal care, light housekeeping, and day-to-day support.
• Support a 2:2 ratio of individuals through a consistent caseload.
• Communicate updates and changes in patient status to the supervising RN.
• Maintain a safe patient environment.
• Provide emotional and social support and compassionate care to patients.
• Maintain current patient records and progress notes.
• Work as a member of the interdisciplinary team.
• Develop and establish a plan of care with a physician.
