Designated Coordinator / LPN (Combination Administrative Role)
About The Position
We are seeking a dedicated and experienced Designated Coordinator / Licensed Practical Nurse (LPN) to join our team in a group home setting. This is a combination administrative and direct care role, ideal for someone who is organized, supportive, and passionate about leading a team while ensuring high-quality care for individuals served. This role blends leadership, compliance, and hands-on support. You will oversee daily operations, support direct care staff, and ensure all MN DHS 245D requirements are met while also assisting with nursing-related duties.
Requirements
- Current LPN license (Minnesota) required
- Experience in group home or 245D services preferred
- Strong leadership, organizational, and communication skills
- Ability to multitask between administrative and direct care responsibilities
- Compassionate, dependable, and team-oriented
Nice To Haves
- Experience with staff training and supervision is a plus
Responsibilities
- Conduct and oversee MAR (Medication Administration Record) audits
- Complete and manage SComms (Staff Communications)
- Provide supervision and support to direct care staff
- Assist with completion of LPN duties as needed during shifts
- Monitor and review ISP data and documentation
- Track and ensure progress on individual goals
- Provide and coordinate staff training, including: Medication administration training, CPR training
- Perform medication administration as required
- Serve as a resource for staff by answering questions and providing guidance
- Help maintain compliance with MN DHS 245D regulations
- Step in to fill open shifts as needed
