About The Position

The Degreed Professional Manager (DPM) is responsible for ensuring that the appropriate level and type of residential supports are provided for all individuals on their caseload at all times. DPMs are responsible for facilitating and ensuring the ability of Lead/Live-in/Host/Direct Care Support Staff for all individuals on caseload to provide a safe, supportive, and therapeutic environment in the home which fosters growth and rehabilitation for supported individuals at all times. The Degreed Professional Manager’s main role is to provide modeling, support, training, and oversight for all staff and individuals on caseload to ensure appropriate supports focusing on the unique needs of each individual served. Degreed Professional Manager will be expected to be available on-call and may be expected to serve as Direct Care Staff when needed and is expected to be familiar with the unique needs of each individual supported, ensuring that all staff/representatives of Mid-Missouri Supported Living are effectively meeting those needs, and notifying CEO, Owner of any concerns regarding abilities in a timely manner.

Requirements

  • College degree or relevant experience.
  • Ability to pass a family safety care registry screening.
  • Ability to show strong work ethic, accept supervision and constructive feedback, as well as provide constructive supervision and feedback.
  • Reliability, punctuality, and flexibility in scheduling.
  • The needs of the individuals being served must be met at all times.
  • Interpersonal skills to engage in appropriate interaction with individuals served, team, and people in the community and/or other agencies and entities.
  • Ability to lift over 50 lbs.
  • Valid driving license, and the ability to drive a van.
  • Ability to complete all agency required training and certifications within 90 days of hire (CPR/First Aid certification, Level One Med Aid Certification, Abuse/Neglect training, etc.) as required by DMH.

Responsibilities

  • Ensure that all staff are trained and comfortable in their roles (direct care and documentation), and able to ensure the health and safety of each individual at all times.
  • Problem solving and providing support when needed for staff and individuals served, training staff on individual’s ISP goals/objectives and ensuring follow through working on said goals/objectives for each individual.
  • Coordinate scheduling of and assist with gathering of information/documentation for annual planning meeting and Individual Support Plans in timely manner.
  • Coordinate/plan/participate in/provide oversight of benefits and programs for individuals on caseload (i.e. social security paperwork as needed, food stamps, Medicaid, Special Needs Trust funding, Special Olympics and other organized participation, etc.), delegating tasks to house staff and ensuring completion as needed.
  • Attend appointments as needed with individuals, staying familiar with needs and follow up timelines.
  • Ensure they are scheduled within timeline needed, ensure coverage, ensure completion, ensure appropriate documentation obtained/filed appropriately.
  • Work with case management on NAFs requests, gathering information needed for those requests as asked.
  • Ensure completion of monthly/daily data collection, or ensure that staff does, ensure quality daily documentation is complete, complete accurate Monthly reviews of services and submit to CMRO.
  • Oversee tracking and ordering of medications/medical supplies as needed and ensure individuals’ needs are met, review MARs on regular basis and ensure accurateness and completeness, ensure proper procedure for medications is followed by all staff and address any issues/additional training needed/medication errors.
  • Spend time with individuals one-on-one as needed and complete required documentation.
  • Ensure completion of daily census record.
  • Ensure all monthly paperwork is submitted in a timely manner on each individual.
  • Ensure individuals’ books in the home are kept up to date and meet DMH requirements and Mid-Missouri Supported Living standards at all times.
  • Work with Administrative team on hiring and training of new staff.
  • Complete new employee checklist ensuring all needed documentation is on file and all needed training is complete, and that new staff are well prepared to meet needs of the individuals they will support.
  • Ensure that calendars of activities, outings, etc. are being made collectively by staff and individuals, coordinate/oversee fair scheduling of transportation for outings and appointments.
  • Ensure home staff are familiar with criteria for DMH Reportable events and that any DMH Reportable events are reported immediately to CEO, maintain regular communication with CEO regarding individuals/homes, also report any problematic behaviors to CEO immediately or within 24 hours.
  • Ensure reports are submitted to CMRO within allotted timeframes.
  • Ensure completion of monthly safety checklist and drills as required by DMH in each home.
  • Create and manage staff scheduling, manage time off requests, be primary contact for call-ins, shift trades, etc. and run overtime through approval of CEO/owner before approving (working with CEO on scheduling).
  • Work with staff and Community RNs to ensure requirements for visits are met, review documentation, ensure any and all nursing delegations are completed in a timely manner, all requests made in monthly reviews are met in the following month.
  • Foster an environment of respect and individual empowerment in the home, providing support, and when needed, corrective action (working with CEO) to enable staff to provide excellent care on a daily basis in the home.
  • Ensure rights of the individuals supported are respected and advocated for and that supports are provided in a way that empowers the individuals supported.
  • Engage in ongoing dialogue with individuals on caseload concerning rights/responsibilities, encourage self-determination and self-advocacy in healthy and socially appropriate ways.
  • Be on call when needed to ensure that Direct Care Staff have supports needed to ensure individuals’ needs are being met at all times and be available when on call by phone and in person if necessary.
  • Submit doctor’s orders for any medication changes to designated administrative team member within required time frame for all individuals on caseload or inform that there are no changes, ensure all homes have MAR for each individual supported by the first of each month to ensure appropriate documentation.
  • Ensure personal items inventory is completed for each individual on your caseload.
  • Ensure one copy is kept in the home, another is submitted for Office Record, and that inventory is updated on a regular basis to ensure accuracy.
  • Amend daily note forms for individuals as needed to ensure current objectives are tracked through questions and submit to CEO for review.
  • Assist with payroll by ensuring house staff collect and submit time sheets of hourly employees by deadline.
  • Keep electronic calendar up to date daily to inform Mid-Missouri Supported Living team of whereabouts/availability when needed.
  • Complete other duties/tasks as asked by CEO/Owner to ensure individuals supported by Mid-Missouri Supported Living are receiving the best care possible.
