DAY SERVICE SPECIALIST

Supportive Concepts for Families (SCFF) is seeking an experienced, passionate leader for the role of Day Program Specialist. Requirements
- High School Diploma or GED
- Six (6) months to one year of experience in Human Services

Responsibilities
- Perform the delivery of services with this organization and our consumers
- Provide transportation of consumers to and from day programming, community activities, and appointments
- Perform all daily activities within day program setting
- Assist consumers with activities of daily living / perform activities of daily living
- Assist with ISP development and outcomes
- Accurate completion of required paperwork to include but not limited to timecards, progress notes, medication logs, vehicle logs, and expense forms
- Be available for shifts twenty-four (24) hours a day, seven (7) days per week as scheduled
- Assist with on call activities and emergency situations as requested by the Program Coordinator
- Responsible for the safety and well-being of all the individuals participating in day services
- Work with Program Coordinator to ensure that all the needs of the individuals are met according to their ISP
- Assist the Program Coordinator with special projects as related to the Agency
- Observes all ATP operating regulations
