About The Position

ADAPT Community Network, formerly United Cerebral Palsy (UCP) of NYC, provides services to people with developmental disabilities. This role is responsible for overseeing daily program operations and providing leadership, coordination, and supervision within an assigned team. The Program Coordinator ensures the delivery of high-quality, person-centered services that promote skill development, community integration, and independence for individuals supported in the Day Services program. They provide guidance to Habilitation Specialists and Habilitation Assistants, ensuring effective implementation of individualized treatment plans, curricula, and activities in compliance with agency and regulatory standards.

Requirements

  • Bachelor’s Degree in Human Services or related field.
  • Minimum of one (1) year of post-graduate experience.
  • At least two (2) years of supervisory experience, preferably in a program serving individuals with developmental disabilities.
  • Strong understanding of OPWDD regulations and person-centered planning.
  • Excellent communication, interpersonal, and organizational skills.
  • Demonstrated ability to lead, coach, and motivate staff in a collaborative environment.
  • Proficiency in Microsoft Office Suite and related applications.
  • Ability to analyze situations, identify problems, and implement appropriate solutions.
  • Must be able to maintain the highest level of confidentiality and professionalism.
  • Strong leadership and management skills with the ability to motivate staff, volunteers, and interns.
  • Establishes and maintains positive and supportive rapport with staff and supported persons as well as with volunteers, aiding in retention and providing coaching as needed.
  • Passion for ADAPT's mission and demonstrates a commitment to the non-profit disability sector.

Nice To Haves

  • Three (3) years of related experience preferred.
  • Valid New York State Driver’s License is preferred.

Responsibilities

  • Provide day-to-day supervision of program activities to ensure services are delivered as designed and are responsive to individual goals, interests, and needs.
  • Supervise, coach, and evaluate assigned staff, including Habilitation Specialists and Habilitation Assistants.
  • Conduct performance evaluations, address performance concerns, and make recommendations regarding disciplinary action, promotion, or termination.
  • Oversee the development and implementation of individualized treatment and habilitation plans in collaboration with clinical and interdisciplinary teams.
  • Ensure documentation accuracy, including treatment plans, progress notes, and other required clinical records, in compliance with OPWDD regulations and agency policies.
  • Develop and implement program curricula, lesson plans, and structured activities that align with person-centered goals and enhance daily living, social, and vocational skills.
  • Conduct regular classroom and program observations to ensure quality, consistency, and effectiveness of service delivery.
  • Support staff development by facilitating training sessions, mentoring team members, and fostering a learning culture focused on best practices in habilitation and person-centered care.
  • Maintain communication with individuals served, families, advocates, and external stakeholders to ensure collaboration and satisfaction with services.
  • Oversee ordering and management of program supplies, equipment, and instructional materials.
  • Assist with the development and implementation of program policies and procedures to promote efficiency, safety, and regulatory compliance.
  • Monitor staff adherence to agency standards and provide corrective guidance or follow-up.
  • Prepare required reports and documentation for submission to leadership.
  • Participate actively in team meetings, staff meetings, and case conferences.
  • Stay informed of regulatory updates, emerging trends, and best practices within the developmental disabilities field.
  • Perform other related duties as assigned.
