About The Position

Responsible for the health, safety, and well being of developmentally disabled clients in Day Program facilities. This role involves creating a learning environment, implementing individual plans of care, monitoring client progress, and ensuring the overall well-being of clients both within the facility and during community outings. The position requires adherence to agency policies, maintaining client privacy, and professional interaction with various stakeholders.

Requirements

  • Must be able to lift and transfer 70-75 lbs.
  • Must be able to utilize approved protective hold techniques.
  • Possess good to excellent oral and written communication skills.
  • Demonstrate thoroughness and follow-through on daily work assignments.
  • Display motivation and interest in improving quality of life for population served.
  • Demonstrate and maintain an optimistic attitude, patience, and compassion for and with the population served.
  • Valid CT. Driver’s License.
  • Ability to drive own and agency vehicle and a good driving record.
  • Public Service License may be required depending upon type of transport vehicle used; Must be obtained within sixty (60) days of hire or transfer; PSL must be maintained for the duration of employment at day program.

Nice To Haves

  • A Bachelor's degree in related Human Services area or an Associate's degree in a related field and one (1) year experience is preferred.

Responsibilities

  • Responsible for the health, safety, and well being of developmentally disabled clients in Day Program facilities.
  • Utilize approved protective hold techniques to ensure safety.
  • Create a learning environment and develop strategies to help clients cope effectively with social, emotional, and physical situations in daily living, and facilitate independent performance.
  • Implement Individual Plan of Care/Follow-Along Plan, record data on teaching strategies and behavior plans, and report client progress, needs, and barriers.
  • Maintain and document awareness of individual treatment plans, including leisure time, community experiences, public transportation use, safety skills, personal hygiene, and activities of daily living.
  • Monitor medical, physical, and behavioral changes in individuals daily and report changes to the Supervisor, R.N. Program Coordinator or Program Director.
  • Write up incident reports and submit to Program Coordinator or Day Program Supervisor within 24 hours.
  • Follow written dietary plans for each client and assist with meal preparation if required.
  • Maintain the appearance, cleanliness, and upkeep of the day program, properties, and vehicle, ensuring they are clean and safe.
  • Report all repair needs to the Program Supervisor and/or Program Coordinator in a timely manner.
  • Maintain clients' right to privacy and adhere to CRI's Confidentiality Policy.
  • Conduct self in a professional manner when interacting with participants, supervisors, co-workers, families, medical profession, and members of the community.
  • Participate in training opportunities as required and maintain all required training per agency policy and DDS regulatory requirement.
  • Take individuals to community-based outings ensuring all health and safety measures are followed.
  • Utilize the agency vehicle following state laws and safety procedures.
  • Provide accurate accounting of agency’s petty cash and client funds in an informative, auditable condition at all times.
  • Familiarize oneself with CRI policies and procedures, ensuring compliance.
  • Perform other work-related duties as assigned by the immediate supervisor and/or other management as required.
