About The Position

Colorado PACE has an immediate opening for a Day Center Supervisor. The purpose of this position is to provide leadership and supervision to staff while assuring compliance with agency policies and procedures. Provides operational oversight of the Day Center.

Requirements

  • Minimum Education: Bachelor's degree preferred. Experience in lieu of education may be considered.
  • Minimum Experience: One year of professional experience required, minimum one year experience working with the frail and elderly population, home care or geriatric experience.
  • Management and Leadership experience preferred.
  • Required License: Valid Colorado Driver’s License and proof of Auto Insurance in accordance with state statute.

Responsibilities

  • Possesses interpersonal skills that promote interdisciplinary collaboration and communication throughout the organization.
  • Supervises Day Center staff in compliance with agency policy and applicable law and regulations.
  • Oversees the scheduling of adequate staff to meet patient/family needs including recruitment, selection and training of new team members.
  • Manages staff performance, including documentation of issues, progressive discipline, and completion of performance evaluations.
  • Models attendance at Colorado PACE events and in-services.
  • Facilitates career development of clinical staff to include orientation and ongoing in-service. Works with clinical educator to provide education to staff as needed.
  • Participates in coaching with supervisor. Seeks guidance, support and encouragement as needed. Keeps supervisor informed of day-to-day operational issues.
  • Assures coordination of activities, nutrition, personal care, clinical service, rehabilitation and other services within the standards and regulations of the program.
  • Assures admission/discharge procedures are followed per regulatory requirement(s).
  • Assures documentation requirements are met per regulatory requirements.
  • Collaborates with other departments responsible for the facility functions and appearance, i.e. housekeeping, dietary, linen, inventory, maintenance, safety.
  • Collaborates with contracting vendors for care center needs, i.e. oxygen, DME, Pharmacy.
  • Understands and ensures regulatory compliance for the programs in the Day Center.
  • Ensures compliance with regulations, professional licensing, and standards of practice.
  • Understands and demonstrates compliance with Colorado PACE policies and procedures.
  • Participates in agency Quality Assessment Performance Improvement (QAPI) Program.
  • Assists with design and redesign of work processes and implementation to improve care.
  • Participates in the development of procedures, clinical protocols, forms and inservice programs as appropriate. Assures appropriate implementation of these items.
  • Develops partnerships within the community and educates community members and healthcare providers about Colorado PACE services.
  • Monitors and responds to reports including: Productivity, Attendance, PI, and DME, Pharmacy.
  • Evaluates staff utilization of pharmacy, DME and supplies and provides guidance/education as needed.
  • Reviews and evaluates requests for approval from accounts payable for patient specific and facility level requests.
  • Monitors staff time and assures accuracy in the payroll system. Follows agency standards with approval or PTO and overtime.
  • Monitors compliance with the approved agency budget, including staffing and direct patient care costs.
