The Data Management Coordinator will maintain existing programs and update data collection processes to meet the goals of the Diabetes Program. This role involves collecting and managing data for analysis of business functions, productivity, and redundancies, ensuring adherence to HIPAA standards. The coordinator will create and maintain the Diabetes Registry using RPMS, including developing search logic with specific codes. They will consult with the Diabetes Program Manager and other staff to develop data collection and storage methods using various programming tools and systems. The position requires attending meetings, representing the department, and providing professional data presentations. Responsibilities include preparing documentation for database systems, instructing users, and creating documentation for program logic and operation. The role involves extensive use of the Diabetes Management System (DMS) for registry maintenance, patient management, report generation, audits, and data retrieval. Additionally, the coordinator will utilize user manuals for DMS and QMan to extract specific data related to diabetes care, including measurements, diagnoses, prescriptions, immunizations, procedures, visits, hospitalizations, and patient history. Support and guidance will be provided to grant-funded departments for data management and extraction under the direction of the Diabetes Program Manager. Accuracy of recorded and stored data is crucial. The position requires using the Electronic Health Record (EHR) to view patient information for updating the DMS registry and generating various data reports for accreditation, grants, quality improvement projects, and other initiatives. Compliance with Privacy and HIPAA standards, as well as CMS Standards, is mandatory. Other duties as assigned.
