About The Position

Avita Health System is seeking a dedicated Survey Specialist to join their Customer Service Department at the Galion location. This role involves performing phone surveys for provider practices, including interviewing, data management (collection, reporting, and trending). The specialist will also manage the Patient Choice Award and Bright Star Award processes, including tracking and managing forms for payroll compensation or gift disbursement. Additionally, this position supports the Patient Experience Director and the Customer Service Program for Avita Health Systems.

Requirements

  • High school diploma or equivalent.
  • Previous customer service experience.

Nice To Haves

  • Associate degree.
  • Minimum of two years of experience in a customer service role.

Responsibilities

  • Performs phone survey process for provider practices: interviewing, data management (collection, reporting and trending).
  • Performs duties connected with the Patient Choice Award and Bright Star Award (tracking and managing forms).
  • Completes process for payroll compensation or gift disbursement.
  • Supports the Patient Experience Director and Customer Service Program for Avita Health Systems.
