The CTI Case Manager provides a nurturing presence, offering active listening, support, and guidance to families and young people facing life challenges in our community. This position ensures that households receive the skills, knowledge, and resources necessary to achieve stability and thrive. This position will involve collaboratively assessing needs, creating personalized support plans, and providing case management services. The CTI Case Manager must demonstrate exceptional empathy, strong interpersonal skills, and a commitment to social justice, ensuring that every individual is treated with dignity and respect. Specialty: Critical Time Intervention (CTI) Case Management Critical Time Intervention (CTI) Case Managers focus on supporting families and youth during the critical period after exiting homelessness, helping them stabilize, stay housed, and avoid returning to homelessness. They provide short-term, intensive support, strengthen natural and community connections, and address barriers that may threaten housing stability. CTI Case Managers are also cross-trained as Housing Navigators, allowing them to assist with ongoing housing needs and help participants maintain long-term stability.
