About The Position

CTI Mental Health Professional (LCSW, LMHC, LMSW, MHC-LP) with a sign-on bonus. The salary posted is for a Fully licensed LCSW, LMHC ($66,770.73 - $69,565.00). Limited permit holders and LMSW are also encouraged to apply, with a salary of $59,789.34 with supervision. CTI is a time-limited, evidence-based care management approach focused on continuity of care for individuals with severe mental illness during transitions from inpatient settings back into the community. This role is part of a unique project partnering Unity of House of Troy in Rensselaer County and Mohawk Opportunities in Schenectady County to provide a multi-disciplinary team approach to supporting participants. The Mental Health Professional (MHP) will provide care management services, therapeutic intervention, psychoeducation, crisis intervention/management plan development, skill development, and other clinical interventions as needed.

Requirements

  • LCSW, LMSW, or LMHC (clinical supervision provided for LMSW/ MHC-LP)
  • 2-4 years prior Master's level experience providing individual therapeutic counseling.
  • Must be comfortable facilitating groups.
  • Previous Experience working in the mental health or substance abuse field is preferred.
  • Valid and clean NYS Driver License.
  • Must have reliable transportation.

Responsibilities

  • Provide psychoeducation and care management services to individuals across both Rensselaer County and Schenectady County CTI participants.
  • Work as a part of a multi-disciplinary mobile care management team.
  • Complete person-centered care planning.
  • Conduct mental health assessments.
  • Provide therapeutic intervention for both mental health and substance use disorders.
  • Provide a range of treatment, rehabilitation, and support services.
  • Develop and maintain relationships with community partners.
  • Complete required paperwork and administrative tasks in a timely and efficient manner.
  • Promote overall wellness and self-management.
  • Promote a culture of inclusion and belonging.
