About The Position

To support the Mission of Helio Health, Inc., the CTI Peer Case Manager will provide comprehensive care management and coordination of services to Critical Time Intervention (CTI) program enrollees. The CTI model is a time-limited form of case management that focuses on helping people with severe mental illness (SMI) and a history of homelessness during a “critical time” transitional period in their lives. CTI is designed specifically to enhance the continuity of care during transition from hospital to community care. Helio Health is on a mission to treat and promote recovery from the effects of substance use, mental health disorders, and other behavioral healthcare issues. To support the mission of Helio Health, the Critical Time Intervention Peer Case Manager acts as a care manager with lived experience, but in addition to Care Management tasks, they resolve medical and rehabilitative needs, when within scope of practice, that impact the individual's ability to meet recovery goals. The Peer Case Manager will work on a Full-Time basis and using a hybrid work model and will cover services in the following counties: Ontario, Seneca, Yates, Steuben, Schuyler, Tompkins and Chemung counties. The schedule is Monday-Friday, 8:00am-4:30pm, with one late shift per week (12pm-8:30pm), and some evening on call responsibilities.

Requirements

  • High school diploma or equivalency.
  • NY State Certified Peer Specialist Certification highly desired.
  • 6 months of Health or Human Service experience with severe mental illness and substance use preferred.
  • Case Management experience is highly desired.
  • An individual does not need to be in recovery themself but must be able to confer empathy through lived experiences such as family and/or close friends (each peer specialist self-defines his or her "peer-ness").
  • Must have a valid NYS driver’s license.

Responsibilities

  • Carry a primary caseload providing CTI services.
  • Develop person-centered care plan.
  • Share knowledge of and provide linkage to resources.
  • Engage individuals in skill building.
  • Offer crisis management.
  • Collaborate with other community providers.
  • Assist individuals in navigating complex systems.
  • Work with service recipients’ informal supports.
  • Other duties as assigned by supervisor.
