About The Position

To support the Mission of Helio Health, Inc., the Critical Time Intervention Mental Health Professional will provide comprehensive care management and coordination of services to Critical Time Intervention (CTI) program enrollees. The CTI model is a time-limited form of case management that focuses on helping people with severe mental illness (SMI) and a history of homelessness during a “critical time” transitional period in their lives. CTI is designed specifically to enhance the continuity of care during transition from hospital to community care. Helio Health is on a mission to treat and promote recovery from the effects of substance use, mental health disorders, and other behavioral healthcare issues. To support the mission of Helio Health, the Critical Time Intervention Mental Health Professional is a care manager, but in addition to Care Management tasks, they resolve clinical and rehabilitative needs, when within scope of practice, that impact the individual’s ability to meet recovery goals. The Clinician will work on a Full-Time basis and using a hybrid work model, working in the community throughout Oneida County. The schedule is Monday-Friday, 8:00am-4:30pm, with one late shift per week (12pm-8:30pm), and some evening on call responsibilities.

Requirements

  • Qualified Health Professional (including but not limited to LMSW, LCSW, LMHC, etc.) required.
  • Minimum of one year of clinical experience in an appropriate program setting.
  • Master’s Degree in a related human service field required.
  • An equivalent combination of training and experience may be substituted for educational requirements.
  • Must have a valid NYS Driver’s License.

Responsibilities

  • Provide therapeutic interventions for both mental health and substance use disorders.
  • Educate individuals about mental health, treatment, and recovery.
  • Teach skills for coping with specific symptoms and stress management, including development of a crisis management plan.
  • developing a relapse prevention plan, including identification/recognition of early warning signs and rapid intervention strategies.
  • Works to create relationships with key providers in the community.
  • Other duties as assigned by supervisor.
