About The Position

North Texas Behavioral Health Authority (NTBHA) is seeking a Critical Time Intervention (CTI) Care Coordinator to join their team in Dallas, TX. This role is responsible for developing a collaborative system of care for individuals with severe mental illness, serious emotional disturbances, or substance use disorders. The Care Coordinator will work with various components of the DOJ Critical Time Intervention (CTI) Team, assisting in connecting referrals, providing community jail release and post-hospital discharge follow-up, and coordinating behavioral health services. This community-based role involves intensive case management over a nine-month period, working closely with community mental health agencies, mental health courts, county jails, and other service providers to address individual needs. Travel is required within the NTBHA Catchment area (Dallas, Ellis, Hunt, Kaufman, Navarro & Rockwall Counties), potentially up to 100% per day, using fleet or personal vehicles for field visits, meetings, and other business needs. The position is exempt and requires schedule flexibility to meet organizational needs. NTBHA is committed to trauma-informed, resilience-oriented care.

Requirements

  • Bachelor’s degree in psychology, social work, or a related behavioral health field.
  • Minimum of two years of experience in community mental health.
  • Experience in providing psychosocial education, skills training, or assessments, particularly with the target population.
  • Experience with participating in a treatment team that develops and monitors treatment plans for persons with severe persistent mental illness and substance use disorders.
  • Knowledge of the Texas crisis response requirements.
  • Possess and maintain a valid driver’s license.
  • Experience in providing services to individuals with substance use diagnoses.
  • Must be able to work on and have knowledge of a PC (personal computer).
  • Advanced Microsoft Office Suite skills - Outlook, Word, Excel, PowerPoint, etc., with an ability to become familiar with company-specific programs and software.

Nice To Haves

  • Experience in telephone or field crisis response teams is preferred.
  • LCDC preferred.
  • Bilingual ability preferred but not required.

Responsibilities

  • Assist in connecting referrals to the program.
  • Provide community jail release follow-up and engagement.
  • Provide post-hospital discharge follow-up and engagement.
  • Coordinate appropriate behavioral health services responses.
  • Provide community-based services to individuals experiencing critical transitions.
  • Engage individuals in intensive case management over a nine-month time frame.
  • Facilitate services through the CTI phases before program discharge.
  • Work closely and collaboratively with community mental health agencies, mental health courts, county jails, and other relevant community services personnel regarding individuals in care to develop effective responses to individual’s unique needs.
  • Use NTBHA’s approved curriculum and assessments with clients.
  • Assist clients while they are in crisis in accessing appropriate mental health services.
  • Make accurate and effective assessments and recommendations regarding the client’s mental health needs.
  • Develop and maintain rapport with persons in crisis using modalities such as the CTI Model, Motivational Interviewing, and other relevant and appropriate methods in order to communicate and relate with callers in order to resolve crises.
