Requirements

  • California Pharmacist License - CA State Board of Pharmacy - REQUIRED
  • AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - American Heart Association - REQUIRED
  • ACLS Certification (Advanced Cardiac Life Support) - American Heart Association - REQUIRED
  • Current license to practice pharmacy in the state of California without restriction or probation.

Nice To Haves

  • Completed a Pharmacy Practice Residency with experience in critical care or a specialized clinical residency in critical care/emergency care.
  • Pediatric Advanced Life Support (PALS) - American Heart Association - PREFERRED
  • BPS Board Certified Pharmacotherapy Specialist (BCPS) - Board of Pharmacy Specialties - PREFERRED

Responsibilities

  • Provides clinical advisory services, including participation in Renal Target Monitoring, Antibiotic Surveillance Team, and Medication Usage Evaluations.
  • Participates in the clinical intervention program by identifying issues and providing recommendations.
  • Responds to and documents drug information requests promptly and professionally.
  • Participates in ongoing data collection for MUE.
  • Demonstrates knowledge and competency of chemotherapy dosing, monitoring, and established guidelines.
  • Reviews patient medication profiles for appropriateness of drug therapy, therapeutic duplications, allergies, dosing, age-specific requirements, renal and hepatic function, and drug interactions.
  • Identifies, resolves, and documents prescribing, formulary, and therapeutic issues.
  • Communicates with nurses and physicians about therapeutic issues or problems.
  • Follows up on formulary compliance issues.
  • Participates in Code-Blue emergencies, mixes and labels medications, and provides information.
  • Assures that pharmacy code-blue billing is completed and restocks medications.
  • Participates in financial pillar/cost savings measures (e.g., IV to PO conversions, formulary adherence).
  • Demonstrates competency in performing formal clinical consults and patient monitoring practices.
  • Participates in Clinical Consult Services (e.g., Pharmacokinetics, Nutrition Support, Ortho Coumadin, Pain Management).
  • Demonstrates knowledge of consult services by attaining consult service goals and proper documentation.
  • Performs by leaving concise, objective, legible, and professional notes in the patient's clinical chart.
  • Completes assigned clinical workload during the shift in a timely manner.
  • Provides comprehensive pharmaceutical care to the medical and surgical ICUs.
  • Prospectively evaluates all medication orders for accuracy and appropriateness.
  • Discusses medication order clarifications with the prescriber and informs others of medication order changes.
  • Monitors critical care drug therapy for appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions.
  • Provides pharmacokinetic consultations and other dosing as required.
  • Participates in the drug therapy management of medical emergencies.
  • Works collaboratively with other clinicians and health-care providers to implement and maintain innovative disease management programs and clinical pharmacy services.
  • Participates in the development of medication use management programs within Sharp Healthcare.
  • Participates in patient care rounds when applicable.
  • Provides drug information to physicians and other members of the ICU team.
  • Provides operational support in the Medical and Surgical ICUs and Central or Satellite Pharmacist as determined by departmental need.
  • Resolves drug distribution related issues.
  • Resolves Pyxis related issues.
  • Expedites antibiotic administration by all appropriate mechanisms.
  • Facilitates proper information transfer with regard to medication use for patients transferred from the ICUs.
  • Promotes the use of the formulary by converting non-formulary orders to formulary when possible, but coordinates procurement of non-formulary drugs when necessary.
  • Promotes and demonstrates rational, cost-effective drug therapy.
  • Focuses on cost avoidance and cost savings due to medication use in the ICU.
  • Monitors the use of expensive medications to assure use consistent with approved criteria.
  • Anticipates changes in drug therapy and reports the potential impact to the overall hospital and drug budget.
  • Reconciles patient home medications, pharmacy and nursing medication records as necessary.
  • Facilitates discharge/admission/transfer for ICU patients.
  • Provides patient-specific medication use teaching for discharge medications when appropriate.
  • Clearly communicates goals and instructions to other staff members, including handoffs between shifts (written/oral).
  • Participates in staff orientation and resident/student training programs.
  • Effectively intercepts and troubleshoots problems, demonstrating effective problem-solving and prioritization.
  • Applies appropriate policies and procedures when faced with complex situations.
  • Evaluates ongoing workflow issues and directs daily workload to achieve efficiency and maximize resource utilization.
  • Attends and participates in staff meetings or reviews staff meeting communications.
  • Reads work-related email at a minimum on assigned workdays.
  • Answers phone in a timely, friendly manner, stating department, name, and title.

  • Dispenses investigational medication according to policies and procedures.
  • Enters data correctly into computerized patient medication profile and verifies technician orders.
  • Demonstrates proficiency with and utilization of automation and advances in technology.
  • Demonstrates knowledge of age-specific medication delivery systems.
  • Demonstrates knowledge and competency of IV Admixture / Aseptic Technique including the standards of USP.
  • Demonstrates knowledge and competency of chemotherapy preparation.
  • Participates in cost management initiatives including managing inventory levels, reducing waste, and using short-dated items.
  • Completes monthly assigned nursing unit / drug storage area inspections on time.
  • Documents discrepancies and makes recommendations for improvement.
  • Brings critical issues to the attention of management.
  • Participates in attaining compliance with the National Patient Safety Goals.
  • Participates and assists in the collection of tracer audits or collection of individual elements from the tracer tool.
  • Ensures security of medication storage and compliance with Medication Management (MM) standards.
  • Takes a leadership role in clinical research and teaching programs related to critical care medicine.
  • Participates in providing didactic and experiential training in Clinical Pharmacy for clinical pharmacy residents.
  • Assists in the development, implementation, and evaluation of residency programs.
  • Actively participates in clinical research projects resulting in presentations and/or publications.
  • Provides staff development support to nurses and physicians.
  • Provides presentations, publications, and other informative activities on critical care medicine and other drug-related topics.
  • Maintains awareness of contemporary trends in the profession.
  • Demonstrates proactive participation in medication safety initiatives.
  • Performs initial analysis of medication safety events and assists in research of causative factors.
  • Performs non-voluntary monitoring.
  • Guides the development of evidence-based treatment protocols, algorithms, and/or clinical pathways.
  • Assists in the development, implementation, and assessment of various technologies used throughout the critical care medication-use process.
  • Maintains compliance with standards of national accrediting bodies.
  • Assists in surveillance and reporting of adverse drug reactions.
  • Participates actively in critical care team meetings and conferences.
  • Participates in ICU-based and hospital-wide committees that impact medication use in the medical and surgical ICUs.
