About The Position

Imagine Pediatrics is a tech-enabled, pediatrician-led medical group dedicated to reimagining care for children with special healthcare needs. They provide 24/7 virtual-first and in-home medical, behavioral, and social care, collaborating with families, providers, and health plans to improve access to quality care. This role enhances existing care teams by offering additional support with compassion and creativity, focusing on children with medical complexity. The primary location for this role is remote, with an expected schedule of shift work (Mon-Fri 8:00 am - 04:30 pm CST or 10:30 am - 07:30 pm CST).

Requirements

  • Master’s degree in Social Work (MSW) Required
  • 2+ years of case management or relevant experience
  • Experience with team-based care, excellent collaboration skills.
  • Experience in child & adolescent case management, school-based care, inpatient setting, and/or community mental health.
  • Experience with acute behavioral health needs (suicidality, homicidality, severe persistent mental illness).
  • Excellent working knowledge of community resources

Nice To Haves

  • Bilingual in English/Spanish preferred

Responsibilities

  • Providing support and encouragement to patients and families who are experiencing or have recently experienced a behavioral health crisis.
  • Handling case referrals, assessing for resourcing needs and care gaps, reviewing case progress, and determining case closure.
  • Assisting patients and families in their individual recovery goals through psychoeducation and linkage to community resources.
  • Assessing social determinants of health and barriers to care including but not limited to housing, income, food insecurity, and transportation needs.
  • Connecting patients and families with support groups, community networks, and aids toward mental wellness.
  • Collaborating with interdisciplinary team members to facilitate the patient and family’s treatment plan goals.
  • Coordinating care management with agencies surrounding the child such as DCFS, school systems, outside care team members, and more.
  • Participating in ongoing scheduled consultations with an interdisciplinary team to monitor patient progress.
  • Performing other duties as assigned.
