Apalachee Center, Inc. is dedicated to assisting individuals and families in North Florida recover from emotional, psychiatric, and substance abuse crises, providing comprehensive behavioral health services across eight counties (Franklin, Gadsden, Jefferson, Leon, Liberty, Madison, Taylor, and Wakulla Counties). The Family Crisis Care Coordinator role focuses on mental health care coordination services, aiming to engage priority individuals and their natural supports who are not yet effectively connected with services. The goal is to ensure service linkage, enhance treatment delivery and recovery supports, and improve outcomes for priority populations. These priority populations include children with serious mental illness (SMI), substance use disorder (SUD), or co-occurring disorders who frequently utilize acute care services (defined as 3 or more acute care admissions within 180 days, or admissions lasting 16 days or longer). The Care Coordinator is responsible for coordinating services, supports, and fostering cross-system collaboration to meet the individual's needs.
