About The Position

The Crisis Behavioral Health Evaluator engages with patients, MRH employees, and other stakeholders using trauma-informed and culturally sensitive communication. This role maintains client privacy and confidentiality, performs mandatory reporting, and assists patients in crafting safety plans to reduce suicide risk. The evaluator conducts and documents high-quality evaluations of suicide risk, mental status, and social needs, providing critical information to medical staff and coordinating care with external partners. They refer patients for treatment to state-approved facilities and coordinate transfers. The position involves recognizing behavioral health concerns, making recommendations, and assisting with behavioral health discharge planning through a multi-disciplinary team approach. Collaboration with discharge planning and care coordination teams ensures continuity of care. The role also contributes to the development of standardized processes, maintains positive relationships with community partners, and utilizes de-escalation skills to manage difficult situations. Attendance at relevant trainings and performance of other assigned duties are also expected.

Requirements

  • Trauma-informed and culturally sensitive communication skills.

  • Proficiency in conducting and documenting evaluations of suicide risk, mental status, and social needs.

  • Understanding of behavioral health concerns including suicidal ideation, substance use/abuse, and social needs.
  • Experience in behavioral health discharge planning.
  • Collaborative skills for working with discharge planning and care coordination teams.
  • Ability to participate in the development of standardized processes and procedures.
  • Skills in developing and maintaining positive relationships with community partners.
  • Exceptional de-escalation skills.
  • Willingness to attend relevant trainings.

Responsibilities

  • Engages with patients, MRH employees, and other stakeholders using trauma-informed and culturally sensitive communication in alignment with MRH Values.
  • Maintains client privacy and confidentiality within the system of advocacy and performs mandatory reporting requirements.
  • Assists patients in crafting safety plans to reduce the risk of suicide, as appropriate.
  • Conducts and documents high-quality evaluations of suicide risk, mental status, and social needs in order to provide critical information to medical staff and coordinate care with external partners.
  • Refers patients for treatment and state-approved 27-65 facilities and coordinates the acceptance and transfer of patients to those facilities.
  • Recognizes behavioral health concerns ranging from suicidal ideation, substance use or abuse, or social needs contributing to overall health and wellness, and makes recommendations or referrals for patients.
  • Assists with behavioral health discharge planning at time of discharge (multi-disciplinary team approach).
  • Works collaboratively with discharge planning and care coordination teams for continuity of care, as appropriate.
  • Participates in the development of standardized processes and procedures that create and support efficient and effective continuity of care.
  • Develops and maintains positive relationships with community partners.
  • Provides exceptional de-escalation skills to help mitigate difficult situations and resolve situations that involve heightened emotions.
  • Attends relevant trainings as needed.
  • Performs other duties as assigned.
