Requirements

  • Six months of experience as a County Caseworker 1; or
  • Successful completion of the County Social Casework Intern program; or
  • A bachelor's degree with a social welfare major; or
  • A bachelor's degree which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of professional social casework experience in a public or private social service agency; or
  • Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.
  • Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits.
  • You must be able to perform essential job functions.
  • You must pass a background investigation.
  • This position falls under the provisions of the Child Protective Services Law.
  • This position falls under the provisions of the Older Adults Protective Services Act.

Responsibilities

  • Responsible for monitoring, linking, and evaluating services.
  • Link to other services and make referrals as needed
  • Provide regular direct individual contact and provide face-to-face case management services while meeting productivity standards as set for the program
  • Schedule and facilitate meetings to develop an individualized service plan with involvement of the individual and/or family
  • Enter necessary information and maintain information in computerized database, state computer systems, and in charts in accordance with state, federal and licensing regulations and Agency policy.
  • Obtain & update information within required time frames
  • Offer crisis intervention services and coordinate assistance as needed.
  • Work with Agency staff, providers, and community resources to support individuals
  • Link individuals to transportation services or transport individuals to appointments as needed
  • Attend meetings, staffings, and trainings as needed.
  • Provide coverage in Somerset County.
  • Perform other related duties as assigned.
