About The Position

The Blended Caseworker is responsible for planning, coordinating, and managing services for assigned caseloads, utilizing recovery-oriented and trauma-informed practices. This position supports children, adolescents, and their families by guiding them through needed services and community supports.

Requirements

  • Two years of experience as a County Social Services Aide 3 and two years of college level course work which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or A bachelor's degree which includes or is supplemented by successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of experience as a County Social Services Aide 3 or in a similar position performing paraprofessional case management functions.
  • Must be able to perform essential job functions.
  • Must pass a background investigation.
  • A conditional offer of employment will require a drug screening.
  • Must submit and have approved satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.

Responsibilities

  • Gather information during initial intake to help enroll individuals in the Behavioral Health Blended Case Management Program
  • Work with families, providers, and the ID Supports Coordinator to link individuals with needed community services
  • Use the Environmental Matrix to assess level of service needs on an ongoing basis
  • Promote resiliency and recovery by following Behavioral Health principles that support everyday living
  • Apply specialized training to support individuals with mental illness and co-occurring intellectual disabilities or autism spectrum disorders
  • Maintain close contact with families and providers to evaluate services and track progress toward treatment goals
