About The Position

This role requires a Master of Social Work (MSW) from an accredited school or a Bachelor's degree in social work with one year of experience in a health-related facility. The position involves charting in an EMR system, preferably Netsmart, and requires strong communication, interviewing, and listening skills. The ideal candidate will have a thorough knowledge of local and state human service agencies, excellent organizational and time management skills, and the ability to work both independently and as part of an interdisciplinary team. Flexibility and adaptability are key. A strong understanding of the hospice philosophy and principles of death/dying is essential, as is the ability to form effective collaborative partnerships with nursing facility staff and demonstrate sensitivity to diverse facility environments. A valid driver's license and a reliable, insured vehicle are also required.

Requirements

  • Master of Social Work (MSW) from a school of social work accredited by the Council on Social Work Education; OR, a Bachelor’s degree in social work from an institution accredited by the Council on Social Work and 1 year of social work experience in a health-related facility.
  • Currently licensed as a social worker in Ohio; must be able to obtain a license in the state of Michigan.
  • Working experience charting in an EMR system, Netsmart system preferred.
  • Strong verbal and written communication skills; strong interviewing and listening skills.
  • Thorough knowledge of local and state human service agencies.
  • Strong organizational, time management, and prioritization skills.
  • Ability to work independently and within an interdisciplinary team.
  • Flexibility and adaptability to change.
  • Understanding of the hospice philosophy and principles of death/dying.
  • Ability to seek, form and foster an effective collaborative caregiving partnership with the nursing facility staff.
  • Awareness and sensitivity to the unique mission and culture of each facility environment.
  • Possession of a valid driver’s license with an automobile in good working order that is insured according to agency requirements.
