About The Position

This role involves providing social work services within an ECF (Extended Care Facility) setting. The position requires a strong understanding of social work principles, excellent communication and organizational skills, and the ability to work both independently and as part of a team. The ideal candidate will be adaptable, knowledgeable about local resources, and sensitive to the needs of patients and facility staff.

Requirements

  • Master of Social Work (MSW) from a CSWE-accredited school OR Bachelor's degree in social work from a CSWE-accredited institution and 1 year of social work experience in a health-related facility.
  • Currently licensed as a social worker in Ohio.
  • Ability to obtain a license in the state of Michigan.
  • Working experience charting in an EMR system (Netsmart preferred).
  • Strong verbal and written communication skills.
  • Strong interviewing and listening skills.
  • Thorough knowledge of local and state human service agencies.
  • Strong organizational, time management, and prioritization skills.
  • Ability to work independently and within an interdisciplinary team.
  • Flexibility and adaptability to change.
  • Understanding of the hospice philosophy and principles of death/dying.
  • Ability to seek, form, and foster an effective collaborative caregiving partnership with nursing facility staff.
  • Awareness and sensitivity to the unique mission and culture of each facility environment.
  • Possession of a valid driver's license with an automobile in good working order that is insured according to agency requirements.

Responsibilities
