About The Position
EverCare Mobile Health is seeking a dedicated and detail-oriented Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Clinical Professional Counselor (LCPC) to join our remote team. We are looking for compassionate providers who are passionate about improving the mental health and well-being of individuals in long-term care settings, driven to make meaningful connections and deliver impactful care to a population that truly needs it. Requirements
- Active LCSW/LCPC License in Oregon (Required).
- Must maintain licensure and remain in good standing throughout employment.
- Ability to pass an enhanced background check through the Department of Health and Welfare.
Responsibilities
- Provide psychotherapy to individuals experiencing psychological, emotional, or behavioral challenges.
- Oversee LPC providers and supervise CBRS workers as needed.
- Facilitate therapy groups and complete necessary assessments (e.g., CDAs) for insurance compliance.
- Develop and implement individualized treatment plans and maintain up-to-date documentation in the agency’s EHR system.
- Collaborate with interdisciplinary teams, including facility staff, PMHNPs, PCPs, and other behavioral health professionals.
