About The Position

We are seeking a detail-oriented Patient Care Coordinator (Scheduling) to support our INVIVA Infusion Services team. In this role, you will coordinate patient appointments, manage referrals, and ensure the timely delivery of infusion and injection therapies. Reporting to the PCC Supervisor, you will collaborate with healthcare providers, pharmacies, and internal teams to ensure patients receive treatment in line with service level agreements (SLAs) and clinical requirements. This role is ideal for someone with strong organizational skills, a customer-first mindset, and experience managing high-volume scheduling in a healthcare or service environment.

Requirements

  • Post-secondary diploma or degree in administration, healthcare, or related field or equivalent combination of education and experience
  • Minimum 2+ years of customer service experience, preferably in a high-volume environment
  • Experience coordinating clients, patients, or scheduling services
  • Strong organizational and communication skills
  • Ability to manage multiple priorities and meet deadlines
  • Computer proficiency (MS Office and scheduling systems)
  • Fluency in English (written and spoken)

Responsibilities

  • Coordinate and schedule patient infusion/injection appointments based on referrals and medical directives
  • Contact patients to book appointments and confirm scheduling details within defined timelines
  • Liaise with pharmacies to confirm medication availability prior to booking
  • Coordinate nurse schedules and ensure appropriate clinical staffing for treatments
  • Maintain accurate patient records and update scheduling systems
  • Manage inbound/outbound calls and emails, meeting service KPIs
  • Send communications (e.g., welcome emails, appointment confirmations) to patients
  • Monitor referrals, follow up on outstanding medication deliveries, and ensure readiness for appointments
  • Handle rescheduling, no-shows, and appointment changes efficiently
  • Collaborate with internal teams and external healthcare partners to ensure seamless patient care delivery
  • Participate in on-call rotation and extended shifts as required
