The Coordinator role is responsible for maintaining an active caseload of individuals referred for START Services. This involves providing systemic consultation based on assessment and outreach findings, utilizing ecomaps, and consulting with systems/teams using best practices. The role requires regular outreach visits to individuals' homes, day programs, or vocational settings for evaluations and assessments, conducted in-person or via telehealth as needed. The Coordinator will assist with referrals for consultation and treatment, including screening for START enrollment and completing intake assessments. They will also coordinate support meetings and crisis plan development, ensuring information is shared with and responses are coordinated with the Medical and/or Clinical Director. The position involves developing Comprehensive Service Evaluations, Crisis Prevention and Intervention Plans, and other necessary documentation. Participation in recurring consultations, staff meetings, and the Center for START Services National Network is expected. The Coordinator will also be responsible for completing and maintaining all documentation, paperwork, and records in a timely manner, accurately entering data into the START Information Reporting System and other required systems. Regular outreach with community partners, developing materials, and presenting at Clinical Education Team Meetings are also part of the role. The Coordinator must complete all necessary trainings to ensure compliance with YAI policies and regulatory requirements, including obtaining and maintaining START Coordinator certification. Compliance with Federal, State, Local, and HIPAA regulations is mandatory. The role also includes on-call support on a scheduled, rotational basis.
