The Coordinator role is responsible for maintaining an active caseload of individuals referred for START Services. This involves providing systemic consultation based on assessment and outreach findings, utilizing ecomaps, and consulting with systems/teams using best practices. The role requires regular outreach visits to individuals' homes, day programs, or vocational settings for evaluations and assessments, conducted in-person or via telehealth as needed. The Coordinator will assist with referrals for consultation and treatment, including screening for START enrollment and completing intake assessments. They will also coordinate support meetings and crisis plan development, ensuring information is shared with and responses are coordinated with the Medical and/or Clinical Director. The position involves developing Comprehensive Service Evaluations, Crisis Prevention and Intervention Plans, and other necessary documentation. Participation in recurring consultations, staff meetings, and on-call coverage is expected. Accurate data entry into the START Information Reporting System and other required systems is crucial. The role also includes conducting outreach with community partners, developing materials, and presenting at meetings. Participation in the Center for START Services National Network is part of the role, along with completing necessary trainings to ensure compliance with YAI policies and regulatory requirements, including obtaining START Coordinator certification. Compliance with Federal, State, and Local regulations, including HIPAA, is mandatory. Performs all other duties as assigned.
