About The Position

This full-time position aims to enhance community behavioral health outcomes by focusing on early identification, engagement, and intervention for individuals of all ages experiencing mental health and/or substance use disorders. The core responsibility is to deliver comprehensive, community-based case management services that support recovery, foster independence, mitigate barriers to care, and prevent the need for higher levels of care. Services include assessment, individualized service planning, referral, resource linkage, care coordination, advocacy, and skill-building, provided in home and community settings using a person-centered, recovery-oriented approach. The role involves collaboration with the Behavioral Health Unit and various community partners such as therapists, psychiatrists, primary care providers, schools, social service agencies, law enforcement, hospitals, detention center staff, the 988 Crisis System, and other local organizations to ensure coordinated care and improved client outcomes. Additionally, the position requires participation in a scheduled on-call rotation for crisis response during evenings, weekends, and holidays, which includes responding to behavioral health crises in conjunction with local law enforcement, emergency departments, detention center staff, the 988 Crisis System, and other community partners. Crisis response duties encompass assessment, de-escalation, safety planning, linkage to appropriate treatment and community resources, and care coordination to minimize unnecessary use of emergency departments, inpatient psychiatric facilities, and detention settings. Employees on call must remain within a 30-minute response radius and work at least one weekend per month. This role necessitates operating State vehicles for home visits, community-based services, client transportation, and meetings. A valid Maryland Class C Non-Commercial driver's license is required throughout employment. Travel to all Worcester County Health Department sites is expected.

Requirements

Graduation from an accredited high school or possession of a high school equivalency certificate
One year of professional experience in assisting in the coordination of a health-related program(s) or referring people to governmental and private resources
Possession and maintenance of a valid Maryland Class C Non-Commercial driver's license throughout employment
Willingness and ability to travel to all Worcester County Health Department sites
Participation in an on-call rotation which includes evenings, weekends and holidays
