About The Position

This position will provide care by helping individuals transition from substance abuse residential treatment to community-based aftercare, recovery-based housing, and support. This position also initiates and supports reentry planning for individuals incarcerated within the local detention center. This position will manage resources for high-risk/high-cost individuals and improve recovery outcomes for those individuals and the addiction/behavioral health care system. This position will connect individuals to recovery resources in their community by providing patient screening, assessment, care planning, referral, and placement.

Requirements

  • Graduation from an accredited high school or possession of a high school equivalency certificate.
  • One year of administrative staff or professional work in a health care setting.
  • Candidates may substitute 30 college credit hours from an accredited college or university for the required experience.
  • Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health related field of work on a year-for-year basis for the required experience and education.

Nice To Haves

  • Experience supporting individuals with behavioral health conditions with a focus on improving recovery and overall health outcomes.
  • Experience with linking consumers to community-based health providers, housing resources, crisis intervention, and other local recovery support services.
  • Demonstrated history of building collaborative partnerships with local stakeholders to assist consumers with essential mental health, substance use disorder, and other recovery support services.
  • Experience with data entry, health record documentation, and creating/submitting reports.
  • Cultural sensitivity, ethical decision-making, adaptability to changing environments, and organizational skills to manage diverse client needs while ensuring compliance with regulatory requirements.
