About The Position

The CDRC Patient Disease Coordinator is responsible for coordinating inpatient/outpatient education for patients with Chronic Disease. This role involves developing and applying evidence-based nursing practice standards to achieve optimal patient outcomes, overseeing evidence-based, individualized, effective care to patients and families in an environment of clinical excellence. The coordinator will assist in maintaining a standardized patient education program regarding their disease, symptoms, and medications, and in the development of a standardized plan of care with measurable goals for patients with chronic disease. They will initiate contact with patients in the acute care setting to provide initial/basic education and encourage follow-up post discharge, serving as an educational resource to hospital staff, patients, families, and outpatient primary care physician staff. The role also includes building relationships with community resources to connect patients with services, implementing and maintaining education classes, and coordinating content experts to deliver education. A disease-specific patient registry will also be maintained.

Requirements

  • Bachelors Nursing required
  • Virginia RN licensure required
  • Ability to obtain licensure in West Virginia required
  • BLS Certification (Basic Life Support) - American Heart ‘Healthcare Provider’ (HCP) - AHA approved required
  • Ability to complete Telehealth certification required
  • Knowledge and skills to develop adult education programs required
